OPS Online Registration

The following forms are required for registration; please complete and return them to the Central Office. Hard copies of these documents can be obtained at the Central Office.

  1. 7-12 OTC Medication Form (Required Grades 7-12)
  2. Letter to Households and Free and Reduced Lunch Application (Optional)
  3. Student Handbooks (For your reference)

If you are enrolling a new student, a birth certificate and copy of immunizations must be provided. The following forms also must be completed: Student Residency Questionnaire (Required for new students), and Home Language Survey (Required for new students).
